Valerie Choniuk is the National Director of Patient Experience at agilon health; focused on transforming health care for seniors by empowering primary-care physicians to focus on the entire health of their patients. She leads enterprise-wide CAHPS and Health Outcomes Survey performance and value based purchasing efforts for Global and Professional Direct Contracting and Medicare Advantage lines of business. She collaborates with leadership, state market executives, and 2200+ community-based Primary Care physicians and advanced practice professionals in 23 independent medical groups, across 12+ states and 25 geographies to focus on our Total Care Model: People, process and technology to improve patient care experience and overall health and well-being for 500,000+ patients.
Her 20 years of experience combine shaping guest, member, customer, employee, and patient experiences from corporate hospitality to across the healthcare continuum: acute to post-acute care, home-health, managed long-term care and clinician groups. Prior to joining agilon health, Valerie was the Director of Patient Experience at CareMount Medical/ Health Solutions, New York’s largest independent medical group, NGACO and MSO, where she led the development of the Office of Patient Experience to provide Customer Service Excellence and quality outcomes throughout the Hudson Valley and New York City. She received her Dual Bachelor of Arts degree in Communication & Rhetoric and Journalism from the University at Albany and a Master of Public Administration and Healthcare Administration Management from Marist College. She is also a Certified Patient Experience Professional through the Patient Experience Institute.
